3. Be sure to include the following:

1.	The form you just filled out.
2.	The original receipt of the item purchased.
3.	Clipped corner from the Book or CD purchased
(enough so that the first 3 digits of the UPC code are visible).
4.	A $1.00 bill or $1.00 Money Order (to cover postage for the CD being sent to you).


Fold this form, tape the UPC code to any area of the form that will not interfere with reading your address or purchase information,
place the dollar bill or money order inside the folded form, place all of this in an envelope and mail it to:
