interactive label form

Hello,
I want to create a form that illustrate the items the user selects. I want to have a columm that list all the items description and respective pictures. Then i need the selected items to be display into specific locations, if the item if not selected then the location will be empty. Each item has a specific location. I'm trying to show others how the label will look like with the information that is require at that time.
