Stacked bar graph with three-month trending

I am trying to make a chart (currently through "pivotchart") that shows stacked bars over three months per goal. For example, one of our doctors has a goal for flu shots, chlamydia screenings, and LDL screenings. So, I want to be able to show how he did over the last three months on every goal. Each bar would be stacked (with successful screenings and failed screenings). In this case, he would have three stacked bars (one for every month) for every goal.

Recap:
3 side-by-side bars per month (last three months) for every goal (three goals for this doctor).
Each bar is stacked with successful and unsuccessful screenings.
