Try this:
=IF(D15="","",LOOKUP(D15,{0,"Low Risk";11,"Low-Med Risk";21,"Med Risk";31,"Med-High Risk";41,"High Risk"})) Those values could be put in a table and then referred to that way rather than contained in the formula as well.
