PLS HELP!!! Review data in Userform

I have looking for days to review data in a userform. Simple adressbook in sheet1:name,adress,city
made a userform with combobox to select name and want excel to show the adress and city for that name. must be simple buth what is the code.........?????????

Please make Userform1.
And make ComboBox1,Label1, and Label2 on UserForm1.

After this, paste following code into Userform1 and Standerd module.
Try.. Run "Sub FormShow()"
