2 Way Look Up using Index Match

Guys,

Can you please help me to solve this? I want to look up D9 and D10 equals to the D13

here is my function "=INDEX(Issuances!$E10:$E$25000,MATCH('Requisition Slip (2)'!$C$1,Issuances!$A$10:$A$10000,0),MATCH(ROW(Issuances!$C$10:$C$10000),ROW(Issuances!$C$10:$C$10000),ROWS(N$3:$O3)))"

I don't see how your question relates to the formula you have tried. There are no references to D9, D10 or D13 in your formula.
What you have attempted in your formula makes no sense logically, it would be beneficial if you post a small, fictional example (10-15 rows of data) in a single sheet that shows both an example of the source and the expected results.
