Access 2007 - Weighting Values

I'm an Access Newbie, I've only gotten into it because our previous Access not-it-all left us. So please forgive me.
I don't know how quite to explain this, if I knew how I'd probably be able to find the answer myself, so I really appreciate any assistance.

I've been asked to setup a database that would summarise the severity level of certain outcomes, with a rating of Low to Very High. Then depending on the individual rating per subject give an average of the ratings. As displayed in the screenshot below.

Does anyone know of a access template that does something like this I could have a look at, or what something like this would be called so I can go on a hunt for one? :)

Excel Facts

Show numbers in thousands?
Use a custom number format of #,##0,K. Each comma after the final 0 will divide the displayed number by another thousand
I would suggest that there is a main table which holds the Field names and data, then you would create a continious form and in the form you can set up either conditional formatting or use code to give you your colour scheme.

To give this ago to start with do something like the table in Excel and look at the coditional formatting or possible VBA code, once you have a better idea how it should work then you should be able to apply this to an Access Form.
