Advice with the table re;ations

Hello World,

I am trying to create a Access Database for use where I work, and a little complicated. I am not sure how to set up the table to later relate them assuming this will be a many to many relations.

I have list of Physicians with the following fields: (first table)
1) PhysicianID
2) LastName
3) FirstName
4) Address
5) PersonalPhoneNum
6) WorkPhoneNum
7) Email

Then I have a 10 different procedures that they need to do through the entire year. This are a few:
1) Encounters
2) PoviderPracticeEvaluation
3) AnomyAvaluation
4) CME
5) ReAdmissionRates
6) Procedures

I have this already created in excel and each sheet contain the information of each Physician and all this procedures as a list and each month of the year. Ex:

NAME OF PHYSICIAN: JOHN DOW
Procedures January Febraury March April May June July August Sept Oct Nov Dec
Encounters 1 1 2 6 9
ProviderPracticeEvaluation 1 1 1 1 1
Etc....

How do I create tables to later relate all this information?

Any Ideas ???

Excel Facts
