Automatic excel generation

Good Afternoon all,
Hope you can help me with this.

This screenshot bellow is a small example of an excel sheet I use to control my fleet maintenance.
In this small example I only have 10 regs and 2 services (PMI and fridge), but if it works with only this I can adapt to more.

L is last service, N is next service, and W is week number for the next service.

I input the last service and the excel automatically calculates the next date. Thing is, I want to have an excel (witch I print) where I put something like this:

I have to input the reg and type of service (if it has 2 services is written right next to it).
What I want is excel to identify what is for service and to write this second screenshot automatically.

I believe it should identify week numbers the same as the next week number, and then find the reg, the type of service and date.

Any way of doing this?

Excel Facts

Bring active cell back into view
Start at A1 and select to A9999 while writing a formula, you can't see A1 anymore. Press Ctrl+Backspace to bring active cell into view.
I would create a pivot table on your layout to extract the data you want. No need to be fancy just select your criteria...week number and other fields
