Building a more user friendly workbook for tracking performance metrics of multiple doctors

I have developed a tool that helps track specific performance criteria on a month to month basis. It is very simplified, but I need to include something that will use yes/no questions of a specific algorithm to build denominator and numerator. Also, I am wondering if I could have a master data sheet for entering the data and If I enter data into a row, can all the data of that row be sent to a specific sheet within the workbook dependent upon the value of a specific cell in that row?
I have columns A through X, column B would be the name of the physician, so I would want all of that data in the row to go to that specific doctor's tab to record. It would also have to be specific to the month in question (column A).
I am not sure if this is making sense, I can see it in my head, but can't get it out in practicum.

Any help would be awesome!
