How do I write a macro for a button to display a user form (from VB) when clicked

For an ActiveX button (from the Control Toolbox) you can use:

Private Sub CommandButton1_Click()
  UserForm1.Show
End Sub

Alternatively, for a macro assigned to a button from the Forms toolbar you can use:

Sub SHowMyUserForm()
  UserForm1.Show
End Sub
