Calculation form - when a product number is entered in one cell then the cost is filled in another cell from spreadsheet 2, what formula?

I am trying to formulate a calculation form for a surgery center that I work for.
What I need is simple, but for whatever reason I can't figure out why it won't work.

For cells B6-B10 if an alpha-numeric code is entered then it refers to spreadsheet 2 for the reimbursement rate $ (also on spreadsheet 2) to be generated in cells F6-F10.

Please help! This will allow us to more quickly figure how much a patient will owe for surgery.
