Call UserForm in a Standard Module

Hi all, I have a UserForm where the name is "DataInput" with the function to allow user to insert data. Now, I would like to include this UserForm into my original module as a small part of the whole program and allow me to process the data obtained from the UserForm. How could I call the UserForm and got access to its data?

The data that I will get from Userform will be a True/False answer only. Thanks in advance for any help.
