Charting week values against monthly target

Guys,

I need to create a line chart from the data below. The users enter 3 values each week, and I also need to plot the predefined monthly target.

The month labels are actually in date format, just converted to mmm

Any help ?

EDIT - Ah, after another phone call . . . what I actually need to chart is the monthly target against the LAST entry for that month from the weekly data.
