Combine 3 worksheets into one

I have three worksheets (Sheet 1, Sheet 2, Sheet 3) with the same number of columns (A:CR) and the same column headers, but with different number of rows. There is no set number of rows in each of the sheets. I need the combined data to go into Sheet 4 starting at cell B2 (instead of A2). In column A, I need the name of the sheet each row came from.

I would appreciate any help. Thanks in advance!
