Concatenate two columns with line break and number them

I have data that looks like this (Column A & B):

Column A	Column B		Column D	Column E
A	First letter of alphabet		A	1. First Letter of alphabet 2. At a birth
A	At a birth		BABY	1. An infant or young child of either sex; a babe
BABY	An infant or young child of either sex; a babe		CLEFT	1. from Cleave. 2. Incised nearly to the midrob; as, a cleft leaf 3. A disease in horses; a crack on the band of the pastern
CLEFT	from Cleave.
CLEFT	Incised nearly to the midrob; as, a cleft leaf
CLEFT	A disease in horses; a crack on the band of the pastern

1. I want a single formula that will concatenate the entire spreadsheet.
2. I do not want data from column A & B in one cell.
3. I want to concatenate as line break.
4. I want the data in each line to be numbered.

Column D & E is what I am trying to achieve.

Please help, been spending countless hours on this lol ?

Cells A235, A3515, 3608 and many others are in error
This might be because you import with a formula strings that began with a "dash", or "equal"
