Control Add Ins

This add in works, but I can't see the icon on my Menu Bar. The only way that I know its there is because when I hold my mouse over that area of my toolbar, the name of the Macro pops up. Are there any commands to add icons and words. I'd like to see a button image and the name of the Macro on the toolbar. Thanks in advance.
