Create columns for one long string 2 rows should be one as well

I have alot of data around 11000 rows and it is all one string. I would like to create columns as I have the locations.
the data looks like this.
NCPDP ID: 3302659 459 Walt Mitman Rd Belville NY 11747-2192 (631) 401-9494 (631) 420-0499 OrderConnect
then the second row is: it looks like some kind of name we can consider it one cell. We would add it to the previous row.

1-17- ID
18-36 - address
37-45 - City
46-47 - state
49-58- Zip
60-73 - phone1
75-88- phone2
89-100 - orderconnect
