creating a button to initiate data input form

I need to implement a macro in a button that will initiate a data entry form. The form will need to have input spaces for each of the columns (names in the top row) of the "2011 Projects" tab. And when completed this form will copy the information into the next open row of the same "2011 Projects" tab. Please help me with the writing of this vba code. Thanks
