Creating a form by pulling out information from a database

I'm really not sure how to approach this problem. I need to creating a form generator that will pull out information from a database and put it on the form depending on whether it is selected or not. I've tried this on excel by using checkboxes and hiding/unhiding rows with certain information, but it isn't very user friendly. I'm wondering if there's something I can do on access that will perform a task like this.

You can create a form direct from a table or query, if you want the source to change then you could create something that will trigger the correct source.
