data from a form to sheet

Hi All,

I have a form which has a calendar control on and also a few textboxes I also a sheet that has dates 1/01/2006 - 31/12/2006 in column A.

I'm trying to get the data entered in the form to be inserted in the against the relevant cell on the sheet (sort of a lookup against the calendar control)

Does anyone have any ideas on how best to achieve this?
