Data Validation for multiple fields in User Form

Hello!

Apologies in advance if I'm posting it in incorrect forum or braking any rules (which I doubt :) )

I'm using Office 2010 Standard and have the following question.

Is it possible to have a data validation from multiple fields in user form?

At the moment my form looks like this

Name
Job Code
Mistakes
Dates x to y

With Name being a name, code being a mixed letters/numbers code, mistakes being a number and date being a date in a format of month,year (eg. May 2015).

I need to do the following:

If the Mistakes number is bigger than 0, then the result from this field would be wrong. If it is exactly 0 then its ok.

I also need to do a lookup on the specified name, part code and dates from a separate spread sheet. The tabs on the spread sheet are named after months (hence the Dates X to Y format of May 2015)

If a person with specified name did the specified job code in specified time period than the result is ok, if not than the result would be wrong.

With this I have 4 possible combinations to show me:

Ok(mistakes)Ok(Job done in specified time by a person)
Ok(mistakes)Wrong(Job not done in specified time)
Wrong(mistakes) Ok( Job done)
Wrong(mistakes) Wrong( Job not done)

I just started experimenting with VBA and currently this is a bit beyond me. I'd like to know whether something like that is even possible
