Database for Continually Changing Data

Hello all-

I am working on a project for evaluating employee phone calls. I created a worksheet, or feedback form, with regular and dependent drop down menus.

What I hope to accomplish, is to find a way to transfer the data from the feedback form to a database of sorts that will house the data collected each month for each representative throughout the year.

Is there a way to automate the whole process using Macros? Below are the cells containing information that I want transferred from the feedback form to a separate table that will act as the database of historical data for each representative.
