Duplicates not removing correctly...

I am trying to help a friend out with a spreadsheet...

I have:

Column A with location names
Column B with Dates
Column C with a Yes value for all records
Column D with "Docs" for the value for all records

What I need to do is remove duplicates so it only shows 1 occurrence of each date for each center.

For example, if I have center 1234 and 5678 that have multiple reports listed under 05/01/11 and 05/02/11 I need to show only one occurrence of 05/01/11 and one occurrence of 05/02/11 (for all centers).

How do I do this? The 'Remove duplicates' button seems to be messing up my data.

All centers should be kept if they occur even once, but only one date occurrence for each location.

So center 1234 could show up a ton of times if they have a report for 5/01, 5/02, 5/03, etc., but if there are MULTIPLES of each date, I want to remove the extra date occurrences.
