Employee grading userform

I am fairly new to user-forms, but here is what I would like to do. I don't know if it is possible in excel.

I have a list of all of my employees
I have a list of categories that the employees grade each other on

I want the employee to choose their name from a drop down list, then I want them to grade each of their coworkers in the 12 different categories, in a user form or a pop up box.

I do not want them to be able to see the results of other employees grading.
