Entering data among multiple rows and columns.

What is the best approach using a userform to accomplish this. My spreadsheet looks something like this:

Description---Control----Keypad----Transformer----Costs

Closet-----------1-----------1-------------1----------$210.00
Garage----------------------1-------------------------$ 69.00
Bedroom--------------------1-------------------------$ 69.00


Userform would ask for a description and then ask if any of the following apply. This would continue until the user had entered in all the needed information.

I don't think we understand what you want. Can you give an example of user input and the result you expect ?

The user would be asked for a description and then asked what equipment would be installed for that area. ie.
Please enter a description - User enters information
What equipment will you be installing - User only needs to put a 1 or an X by the equipment to be installed.
