Excel Create a User Form

Hi all and thanks in advance for viewing my thread!
I have created a spreadsheet that will be used by various users. The idea of it is that a pdf presentation is created based upon the inputs (responses) entered by the user on a questionnaire form. This form currently is just a worksheet with entered cell values as questions and an area in adjacent cell for the answer.
What I am hoping to accomplish is instead of a normal excel worksheet acting as the data entry form for the user, I would like it to appear more "program-like" and have an input box appear that asks the questions and has an area for the user to input the answer.
If anyone is willing to help out I would very much appreciative! Obviously, I can provide more specific information after the fact. Thanks so much!
