Excel Formula of multiple column

Hy, Please help me with this.
I want to have formula for below:

if A3 date is greater than date 31 jun 2014 and b3 value is "JWV" and c3 date is between 21 oct 2014 to 31 oct 2014 then the D3 value has to be "SPO YES"

Please make a formula for the above.
