Excel Receipt

I looking to a make a receipt for prove of payment in excel, but that is linked to another sheet so i can use the info on the receipt to create a report( first, last name, amount paid,school,)it for a soccer show case event. Maybe someone has something like already. Or maybe point me in the right direction
