Excel -> Word Mail Merge Automation?

Hi, I don't know if this is possible... just wondered if there's something like like this i can do.

I have a data entry form in Excel which then enters data into a database (see below)

I then have a form in which this data will be inputted.

What is the best way of automating this process?

I know you can create a mail merge (i've done this). Is it possible to create a button on excel in which automatically opens this form and enters the desired data?
 

Excel Facts

What do {} around a formula in the formula bar mean?
{Formula} means the formula was entered using Ctrl+Shift+Enter signifying an old-style array formula.
For further info as i am yet to get a response,

I would like to click on a S.No (as these are unique) and then play a macro that would copy just this line of data into the table on Word.
