Fill in a field based on a lookup?

I am trying to create a table that houses when a person makes a request. I have one field where they select their name from a drop-down list. What I would also like to do is have fields update with that person's phone number and email address. I have everyone listed in a table with contact info. Is there a way to get a field in a table to perform a lookup in a different table to do this?

This is what I tried so far, but all it does is create a drop-down list and it seems to only include the first email address in the contact table. Is there another way to do this?

Create a combobox with three columns: Name, Phone Number and email address using the contact info table. On the On_Click() event procedure you can insert phone number and email address into their respective fields with a small vba routine that will run when the user clicks on the combobox to select the name. Sample program is given below:

Private Sub EmployeeCombo_Click()
     Me![Phone] = Me!EmployeeCombo.Column(1)
     Me![Email] = Me!EmployeeCombo.Column(2)
