Form Creation. Contents should show in the form based on Validation

Hi,

I need to create a form in access, and the contents in the form should reflect based on the validation that is selected.

Eg.

If the form begins with the check List: Like Bank Name, Post Office, Website

When i select the a particular check list eg. Bank Name,

It should give me the all the fields under bank name, like bank account#, Routing #, Che#.
