Form to upload data into SQL Server

Dear Friends,

Could anyone do me a favor in creating a form which can take data from a sheet and upload into SQL Server (My SQL Server is not in local machine , Its a remote machine)
I will keep Database credentials within VBA code only. Thank you in advance. below is sample of a form

Excel Facts

Remove leading & trailing spaces
Save as CSV to remove all leading and trailing spaces. It is faster than using TRIM().
What does Sheet1 look like? If the SQL Columns names are in some row, then I would write code to walk Sheet1 and write the data directly to SQL Server. I do something similar, I have all/select records/columns showing for a table and allow the user to make changes to multiple columns and multiple records and send all the updates to SQL Server via a single button with VBA code that creates multiple SQL Update Query(s).
