Formating a template to look like a form

Okay, so I thought I was pretty good with excel, but I am having a hard time with this one. I have finished an excel template that I would like people to be able to use a a fillable form. Everything is set up as a true template and ready for use; however, when you open it, it looks like an excel spreadsheet rather than a form. I went as far as to start to completely recreate it in adobe, but I figured I would ask if this is something that can be done relatively easy in excel. Again, it already functions in the way I want it to, but it just does not look as professional as I would like it to.
 

Excel Facts

The =SQRT(25) is a square root. For a cube root, use =125^(1/3). For a fourth root, use =625^(1/4).
