Formula for defining range based on criteria

I am creating a football sports model in Excel. I have a list of the "home" teams in column A and the "away" teams in column B (each row depicts one match between 2 teams). Because this is an entire season, there are obviously several duplicates of the teams in the columns (as a team plays more than once in a season). This also means that all teams will appear more than once in both columns. I am looking for a formula to define the range of a team's last 5 games in the season, so that when I add more matches, it adjusts. Thank you in advance to anyone that can help!!

More specifically, the question I'm trying to answer with this formula is "what is the range of *team*'s last 5 matches.
