Formula in Pivot Table from a Scale within a Field of the Data Set

I am new to this forum and tried searching but eventually gave up - so I do apologize if this has been asked before. :(
I have survey data in MS Excel. The majority of the questions were scaled from 1-4 and then there is a 0 for no response and 5 for 'I do not know'. I am trying to total the 1+2 and 3+4 responses but these are not options in the fields given in the dialogue box when you try and add a calculated field - only the question itself is. Without having to go to formulae in separate tables taken from the pivot tables, is it possible to add this calculated field from the scales of the questions and then use this calculated field for a chart? I did try various ways but to no avail. Any help will be greatly appreciated please.
