Formula that returns the count of items that meet two criteria

I'm hoping someone can help me because I've been searching the web and beating my brain over this for a day now.

I have a spreadsheet where I need a formula that will count how many sites have a document type "Lease Review" that are due between 9/1/14 and 9/30/14 only. The result that should come back is "2", but I can't seem to get any formulas I have found to work out correctly. A sample of what I'm working with is below. Any help would be greatly appreciated.

I will give it a try and do a few test runs on it and let you know. I've never used TEXT before and haven't seen any formulas that included it, so I never even considered it. Thank you!
