Formula to track multiple activity per account and 2nd formula for MOM activity per account

Hello all,
I've got a pivot that is providing me with the number of times a rep has backdated an account. I have been unable to find the right formula to capture any months where there has been backdating done multiple times within the same month. I've also tried to figure out the right formula to use in order to identify reps and accounts where backdating has been done over multiple months with no luck. I think the best formula would be a countif formula for the first part, but each time I've written it, I either get an #N/A or an error message indicating my formula is incorrect. I haven't tried to figure out the formula for the 2nd part, because I've been stuck on the 1st part. Because I'm unable to attach anything, I've pasted an example of the output I'm trying to get based on the data within the pivot table. Any and all help is greatly appreciated. Thanks!
