Help producing a calendar which displays information in pop up form for each day?

Hi Folks,

I've been asked to create a diary type document for recording when certain documents are due for review. What I would like to do is have a caldendar type format, which changes the month being displayed when the user changes themonth from a drop down- this much is easy enough, I've downloaded a template for it in fact.

The next part though is the tricky bit- I would like a pop-up form to be produced which contains information pertinent to the docuent due for review, and for this form to be saved to the relevant day. So what needs to happen is that the pop up form is saved to the day on the calendar, this way it can be referenced in the future. The form doesn't need to enter data into another table once completed, it just needs to retain the information entered into it.

Is this possible?

So process again is, for clarity:
  1. select day from scrollable calendar
  2. press a button which opens a blank form uniquely attributed to that day
  3. enter data into form
  4. save form
  5. form remains when scrolling onto other months
  6. form can be edited whenever required
So far I have found a calendar template which provides the basis for a scrollable calendar, and have just about worked how to create a user form- however everything else, I am yet to work out.

Any help, as ever, is greatly appreciated.

Edit: The calendar template does not store data once you navigate to/view other months. So this would need resolving as a starting point to begin with!
