Help with date

Hello,
this is a very basic question but I can't find the solution.
How can I get in the cell H3 a value like 1-jan or 2-jan etc. if I change the month in H1 it has to change in cell H3 also to the same month as H1.
The formula in H6 is =INDEX(C2:C5,MATCH(1,(H3=D2:D5)*(H4=A2:A5)*(H5=B2:B5),0))
I hope someone can help me?
