How do i change the first table to second table?

This is the table i have.

A	A1	B	B1	C	C1
D	D1	E	E1	F	F1

This is the table i want.

Name	Code
A	A1
B	B1
C	C1
D	D1
E	E1
F	F1
G	G1
H	H1
I	I1

I want to change the first table to second table. How can I do it? Any help with the formula or VBA would be a great help. Thank you!
