How do I filter pivot table/ slicer by gender?

I have survey data in a table with the first column header being 'Question,' and each row below it is a survey question. Each cell contains the participant response (in text).
Since there are many responses, my pivot table allows the user to filter by question to review the corresponding responses. However the very first question is 'Gender', and I'd like to have a slicer that allows the user to quickly filter the survey question and then a separate slider to filter by gender.
I'm having trouble doing this seeing as gender is in a question row and not a table header.

Tough to visualize your issue. Suggest you post sample data (8-10 rows) of what your source data looks like (anonymized) and then what you want your pivot table to look like (mocked up).
