How do you take a list of data and put it in columns?

Example - I want to make this:

NAME Deduction Amt
John Medical 66.55
John Dental 83.19
John 401K 499.15
John 401KM 99.83
Mike Medical 43.06
Mike Dental 53.83
Mike 401K 129.18
Mike 401KM 64.59

look like this:
Name Medical Dental 401k 401km
John 66.55 83.19 499.15 99.83
Mike 43.06 53.83 129.18 64.59
