How to insert a datasheet into a form the contains varying data?

I am trying to do this with a split form but it will not let me add different fields on the form side.
 

Excel Facts

Show numbers in thousands?
Use a custom number format of #,##0,K. Each comma after the final 0 will divide the displayed number by another thousand
