Hyperlinked IF function?

Hello, is there a way to use the hyperlink function along with the IF function?

For example, I was a block of text to take you to a certain sheet based on the value in another sheet.

LINK Text – “P&L Worksheet” (If the value in sheet 1 is 12; it would take you to another sheet labeled 12, if it’s 13 it would take you to sheet 13, etc)

Hope this makes sense
