If, And, Or formula with 1 true and 2 false

Hi - I am getting an error on this formula and I don't know what I am missing. Please help:IF(AND([customer form completed]<>"",OR([Fee
PD]="Yes",[Fee PD]="N/A")=FALSE,[Fee PD]="WAIVED")=FALSE)=TRUE,"Collect Fee")
 

Excel Facts

Select a hidden cell
Somehide hide payroll data in column G? Press F5. Type G1. Enter. Look in formula bar while you arrow down through G.
What's with all the =FALSE, what are you trying to check for exactly? You also have nothing for it to do when FALSE.

This is a guess of what you want. In this sample A1 is customer form completed and B1 is Fee PD

=IF(AND(A1<>"",B1<>{"YES","N/A","WAIVED"}),"Collect Fee","")
