iinsert row + copy page + rename the page to get the same value of the last name

Hi I am creating a form for a paying time sheet

I would like toadd two thing to spred sheet by the VBA

the frist one is :

Insert row
pop window ask for 4 thing
1 : first name will go to cloum A
2: last name will go to cloum B
3: ID number wll go to cloum C
4: Position number will go to cloum D
Then
copy page name orginal and rename it to match the last name
and rearrange the rows by the last name..

2: click to delete the row and the page assign to it.

Ok I did the insert thing, but is there anyway I can the page and make it the same as the last name ..!
