Importing Appointments from Excel into Outlook then Email Reminder

Hello, I have a Excel spreadsheet with a list of names and dates that I want to import into Outlook as Appointments. The problem is that each name has a 3 different appointment dates i.e.
Name
Ins 1
Ins 2
Ins 3
Email
Dr X
20/04/2013
20/05/2013
20/06/2013
stewart.aitken1@btinternet.com
Dr Y
25/04/2013
25/05/2013
25/06/2013

Is there a way to import these multiple dates into Outlook or do I have to separate them into single dates manually? I have a long list so this would be very time consuming. I can only do it for single dates using Outlook -> Import Excel 97-2003.

I also want an email reminder sent automatically using Outlook instead of the pop-up reminder. I have used my own email address in the above example but the actual email addresses would be different for each name. How can I do this?
