Index column of dates Macro with prompt?

I have a column full of dates (column d).
I need to be able to index all the dates in the column by a specific number of days forward or backward.

I'm trying to create a macro that will prompt me for the number of days I wish to index the dates in the column, and also whether to index forward or backward depending on which radio button is selected. Default would be forward. Then, when I enter the info and click OK, it's done.

Any help would be greatly appreciated!
