index match help

I am trying to pull in a value based on 3 variables. 1-a specified date which is entered into a ref cell (D2) and specific shift to return the value listed under "Bulk Schedule"
the below is what i am trying to use but it is returning "#name?"

any help or suggestions on how to make this data pull formula work???

You could use a formula a little like this

=INDEX(A2:E11,MATCH(1,(A2:A11=D2)*(B2:B11=1),0),MATCH("Bulk Schedule",A1:E1,0))

Needs to be confirmed with CTRL+SHIFT+ENTER so that curly braces like { and } appear around the formula in the formula bar
